Pain at the front of your hip is often described as a deep “pinch” with squatting and bending based movements.
This is commonly called “hip impingement” or “Femoroacetabular Impingement” FAI.
The hip joint is made up of the femoral head (ball of the hip) and the acetabulum (cup of the hip). FAI is a motion related clinical injury that causes symptoms, clinical signs and sometimes findings on imaging.
Symptoms can include sharp, catching, pinching sensations at the front of your hip around your groin and top of thigh. It can often be felt at the bottom of the squat, climbing stairs and prolonged sitting.
Reduced range of motion, stiffness, clicking, catching, giving way, decreased ability to perform activities of daily living and sports are other symptoms considered for FAI.
FAI is multifactoral but can be split into two factors, intrinsic and extrinsic.
Intrinsic factors include differences in morphology (shape of bones), history of childhood hip disease. Weakness of the deep hip muscles that help with control and stability.
Extrinsic factors include exposure to repetitive hip rotation and flexion movements, previous hip surgery.
Different hip morphology is common around 30% of the general population, including people without symptoms.
Surgery is sometimes required for FAI, however, research has shown only short term benefits.
A full assessment is required to find areas that need improvement. We will look at pelvis mobility and control, hip range of motion and strength of the surrounding muscles and structures.
The most effective form of treatment for this condition is a combination of strength and mobility exercises, mobilisation of the affected and surrounding areas, and education of how best to manage your pain.
FAI and pain at the front of your hip is not a quick fix. It will often present with a triad of symptoms. Conservative treatment is the preferred first option for treating your pain with an emphasis of load management and activity modification.
